November 2011: DEPENDENT ELIGIBILIGY FORMS
Please fill out these forms as they pertain to your dependents at this time. Eligibility requirements are as follows:
For HEALTH / MEDICAL Coverage:  An employee’s dependent child can be covered to the day of their 26th birthday for MEDICAL coverage, without regard to support, residency or other dependency factors.
For VISION and/or DENTAL Coverage:  A dependent age 19-25 can remain on your VISION and/or DENTAL coverage if they meet the following criteria: · Must be a member of your household · A full-time college student five months out of the calendar year (they can be living at school) OR · Earning less than four times the IRS personal exemption amount · You must be providing the majority of their support. Currently, the maximum length of time a 25-year-old can remain on your VISION/DENTAL coverage is the end of the calendar year in which they turn 25.

November 2011: FLEXIBLE SPENDING ACCOUNTS
Members should note the following due dates for their 10-11 Flexible Spending Accounts:
10-11 Dependent Care claim reimbursements are due by 11-30-11.
10-11 Health Care expenses may be incurred up to 11-15-11.
10-11 Health Care claim reimbursements are due by 12-30-11.
After these due dates, all monies remaining in accounts will revert back to the school district and the PCEA Scholarship Fund.  $5467.20 was left over and deposited into the PCEA Scholarship fund from the 09-10 year.  This was ½ of the total monies left over. 
$300 was loaded onto your FSA Benny card on 9-1-11.  New cards were mailed in August for members not previously enrolled.  Do not destroy Benny cards; it will cost to replace one.  
Eligible FSA health care expenses include:  Deductibles · Co-pays · Office visit co-pays · Well baby care · Immunizations Psychologist · Physical Therapy · Chiropractor · Orthodontics · Eye exams · Glasses / Contacts · Hearing Exam / Hearing Aid · Rx co-pay · Acupuncture

November 2011:  Life Insurance
The previous teacher's life insurance policy that was provided by P-CCS was terminated on November 1, 2011.  Voluntary life insurance can be purchased through MetLife.  Additional information on purchasing voluntary life insurance can be found on this website or you can contact Dawn Schaller.


 
  The PCEA Benefits Committee consists of teacher representatives from each building in the Plymouth–Canton School District. Patti Wert is the PCEA Benefits Chairperson and can be reached at (734) 981-3363 or patti.wert@pccsmail.net. The purpose of this committee is to gather information regarding all employee benefits (health, dental, vision, long term disability, short term disability, flexible spending accounts, employee assistance plan, family medical leave, and HIPPA privacy practices) and make it available to PCEA members. PCEA Benefit Committee representatives are available to all PCEA members to answer questions and/or assist members in getting benefit questions answered. Dependent Eligibility for 19-25 year-olds for Dental & Vision coverage only

The dependent must:

A. Be a member of the employee’s household AND

B. Be a full time college student 5 months of the calendar year OR earn
less than four times the current personal exemption amount allowed
by the IRS.

C. College students may live away from home.

D. The end of the calendar year, for 25 year-olds, is the maximum
length of time they can remain on your coverage.
