NEW BLUE CROSS BLUE SHIELD CARDS - Teachers should be receiving new Blue Cross Blue Shield cards in the mail that will be effective July 1, 2010.  This card does NOT represent a change in your benefits.  It is only being issued because Blue Cross is now using Medco Health Solutions to process your pharmacy claims.  Therefore, as of July 1, you should show your new card when filling any prescriptions.  You do not need to inform your doctors that you have a new card, as your coverage has not changed. 

FLEXIBLE SPENDING ACCOUNT - Teachers who would like their Flexible Spending Account reimbursement checks directly deposited into a bank account may fill out a "Direct Deposit Authorization Form" found in the Flexible Spending Account section below and send it to Meritain Health at the address listed on the form.

OPEN ENROLLMENTS HEALTH, DENTAL AND VISION - May 3 to May 28, 2010 This year’s open enrollment period for health, dental and vision will be the month of May. Memos will go out to all employees the last week of April with information on open enrollment. If any changes or additions need to be made to your health, dental or vision coverage, or you are eligible and would like to enroll, please contact Dawn Schaller (McBeath). She will send you enrollment/change forms.

FLEX SPENDING ACCOUNT OPEN ENROLLMENT - Entire month of June 2010 (Information will be sent to the buildings in May 2010)

SOCIAL WORKER REIMBURSEMENT - Effective immediately, all requests for social worker reimbursement can now be faxed to BCBS. Please attach a cover sheet with your request to show that you are from Plymouth-Canton Community Schools and fax your information to 1-866-571-3746. This fax line is the direct line in the department that is processing our social worker reimbursements. This will speed up the reimbursement process. The PCEA Benefits Committee consists of teacher representatives from each
building in the Plymouth–Canton School District.  Patti Wert is the PCEA Benefits Chairperson. and can be reached at (734) 981-3363.  The purpose of this committee is to gather information regarding all employee benefits (health, dental, vision, long term disability, life insurance, flexible spending accounts, employee assistance plan, family medical leave, and HIPPA privacy practices) and make it available to PCEA members.

answer questions and/or assist members in getting benefit questions answered.

Dependent Eligibility for 19-25 year-olds

            The dependent must:

A.     Be a member of the employee’s household AND

B.      Be a full time college student 5 months of the calendar year OR earn           
less than four times the current personal exemption amount allowed             
by the IRS.

C.     College students may live away from home. 

D.     The end of the calendar year, for 25 year-olds, is the maximum             
length of time they can remain on your coverage.
